    Assisted Living

    Sterling Park Assisted Living offers the comfort and convenience in a community with a wide variety of living options, home care services and amenities. You will no longer have to worry about snow removal, lawn care and daily upkeep of a home. At Sterling Park Senior Living, you will be living with a minimal or moderate level of assistance. We value Person Centered Care. As your needs change,so can the services we provide. Personalized and comprehensive health services are available as your needs change. No two people are alike, and the same holds true for the services we provide to each resident.

    With our commitment to the person, we focus on our 7 Dimensions of Wellness to meet the goals and needs of those who choose to call Sterling Park “Home.” You can expect their will be opportunities to socialize with friends in the Bistro, participate in card or book club, attend worship services, work out in the fitness room or attend an exercise group. At Sterling Park, we do everything that we can to ensure you have a productive and comfortable recovery after a traumatic experience or scheduled surgery.     Outpatient Therapy

    Outpatient therapy is a form of therapeutic treatment offered to individuals who do not need to be in a skilled nursing setting on a continual basis. A number of therapies can be offered on an outpatient basis including occupational, speech, physical, and post-surgical care. Many patients prefer outpatient therapy because it allows them to receive necessary medical care while staying at home. It also allows patients to live a relatively normal life while simply setting aside time to attend therapy sessions.

    Assisted Living

    It’s not always easy to tell when you or a loved one needs more help. The following warning signs
    may indicate that it’s time for a talk about assisted living.

    • Do you find the refrigerator is empty or filled with spoiled food or you or your loved one
      are losing weight? These may be signs of not eating well because shopping or
      cooking is difficult.
    • Do you notice frequent bruises? This may be a sign of falling or mobility and balance
      problems.
    • Does your loved one wear the same clothes over and over again or neglects personal
      hygiene? This can indicate that doing laundry and bathing is physically challenging.

    Respite and Recovery

    Everyone needs a little respite!

    Our respite program is available for a 7 to 60 day stay. When informal caregivers require a leave or if you are recovering from illness, injury or disease, we have a place for you to stay. Due to the short-term and/or acute nature of the service, it is offered as a complete, individualized package including:

      • Furnished apartment
      • All utilities (except phone)
      • Three meals per day
      • Housekeeping
      • Weekly linen and personal laundry
      • Emergency response system
      • Group activities
      • 24 hour staffing
      • Wellness checks
      • Initial assessment and monitoring by trained staff
      • Assistance with personal cares such as bathing, dressing, grooming, toileting, eating and mobility
      • Medication administration
      • Medication set-up
      • Nurse supervision and monitoring by trained staff

    There is a per day cost for respite care services; please call for rates. Discharge is anticipated within
    60 days, or an assessment by one of our registered nurses is completed to determine current status and make recommendations for appropriate service options. Other skilled services, such as physical therapy, will need to be purchased from the health care provider of your choice.

    Visiting Physicians

    Partnerships are crucial to providing the best and most comprehensive options for our tenants. Onsite physicians can take away the stress of fighting weather and traffic to get to your appointment, if you choose. This is an approach where primary care providers, families and patients work in partnership to improve health outcomes and quality of life for individuals with chronic health conditions or disabilities. This team approach to care gives you access to all the services and support you needed.

    Our experienced team has been committed to quality improvement through innovation and alignment with our core mission. Listening and responding to the needs of our tenants and families is imperative to maintaining our commitment to quality care. One area of ongoing concern in healthcare is medication management —specifically medication errors, overuse, and thorough, quality review.

    In response to this need we’ve come together with pharmacy experts utilizing an affiliate organization to meet our needs. TotalCare Pharmacy will provide extensive benefits, with pharmacy services tailored to our processes, allowing Tealwood managed and owned properties to provide you the best care possible.
