    Assisted Living at Sterling Park Senior Living offers comfort and convenience in a community with a wide variety of living options, home care services, and amenities. You will no longer have to worry about snow removal, lawn care, and daily upkeep of a home. At Sterling Park Senior Living, you will be living with a minimal or moderate level of assistance. We value Person-Centered Care. As your needs change, so can the services we provide. Personalized and comprehensive health services are available as your needs change.     Transitional Care

    Sterling Park Senior Living offers our patients state-of-the-art rehabilitation services. We offer Physical, Occupational, Speech, and a wider array of specialized therapies on an in-patient and in many cases, outpatient basis. At Sterling Park, we do everything that we can to ensure you have a productive and comfortable recovery after a traumatic experience or scheduled surgery. Our goal is to get you back to where you call home.

    Outpatient Therapy

    Outpatient therapy is a form of therapeutic treatment offered to individuals who do not need to be in a skilled nursing setting on a continual basis. A number of therapies can be offered on an outpatient basis including occupational, speech, physical, and post-surgical care. Many patients prefer outpatient therapy because it allows them to receive necessary medical care while staying at home. It also allows patients to live a relatively normal life while simply setting aside time to attend therapy sessions.

    If you suffer from any of these conditions, you could benefit from outpatient therapy:

    Respite and Recovery

    Everyone needs a little respite!

    Our Assisted Living respite program is available for a 7 to 60-day stay. When informal caregivers require a leave or if you are recovering from illness, injury or disease, we have a place for you to stay. Due to the short-term and/or acute nature of the service, it is offered as a complete, individualized package including:

    • Furnished apartment
    • All utilities (except phone)
    • Three meals per day
    • Housekeeping
    • Weekly linen and personal laundry
    • Emergency response system
    • Group activities
    • 24-hour staffing
    • Wellness checks
    • Initial assessment and monitoring by trained staff
    • Assistance with personal cares such as bathing, dressing, grooming, toileting, eating, and mobility
    • Medication administration
    • Medication set-up
    • Nurse supervision and monitoring by trained staff

     

    There is a per-day cost for respite care services; please call for rates. Discharge is anticipated within 60 days or an assessment by one of our registered nurses is completed to determine current status and make recommendations for appropriate service options. Other skilled services, such as physical therapy, will need to be purchased from the health care provider of your choice.

    Our experienced team has been committed to quality improvement through innovation and alignment with our core mission. Listening and responding to the needs of our tenants and families is imperative to maintaining our commitment to quality care. One area of ongoing concern in healthcare is medication management—specifically medication errors, overuse, and thorough, quality review.

    In response to this need, we’ve come together with pharmacy experts utilizing an affiliate organization to meet our needs. TotalCare Pharmacy (at our Assisted Living) will provide extensive benefits, with pharmacy services tailored to our processes, allowing us to provide you the best care possible.
