Prairie View Senior Living provides care for individuals when staying at home is no longer possible. We provide a person centered environment to meet the needs of those who choose to call Sterling Park home. In addition to rehabilitation services, we take the worry and stress out of daily living with the following benefits. We know that each person is unique, and we want to help meet the individual goals and needs of our residents.
Prairie View Senior Living offers our patients state-of-the-art rehabilitation services. We offer Physical, Occupational, Speech and a wider array of specialized therapies on an in-patient and in many cases, outpatient basis. At Sterling Park, we do everything that we can to ensure you have a productive and comfortable recovery after a traumatic experience or scheduled surgery. Our goal is getting you back to where you call home.
Outpatient therapy is a form of therapeutic treatment offered to individuals who do not need to be in a skilled nursing setting on a continual basis. A number of therapies can be offered on an outpatient basis including occupational, speech, physical, and post-surgical care. Many patients prefer outpatient therapy because it allows them to receive necessary medical care while staying at home. It also allows patients to live a relatively normal life while simply setting aside time to attend therapy sessions.
Q. What factors should be considered when choosing a skilled nursing and rehabilitation community?
A. It is important to select a community and team of care providers that can offer the appropriate care that you or your loved one requires. Not all communities offer the same medical services. If your care plan will call for wound care treatment or physical therapy, as examples, you will want to select a community that offers access to these programs, on-site. It is important that you also select a community that feels like the right fit for you or your loved one. Take some time to visit the communities you are considering. Take a tour and meet with the staff. Choose the one that can best accommodate your medical needs, but also feels comfortable and can serve as a true home away from home.
Q. What is a plan of care?
A. A plan of care identifies the specific steps that will be taken to treat and care for a patient. Plans are individualized and based treatment that will achieve the best possible outcomes for you or your loved one. Physicians and other members of your clinical care team will work collaboratively to develop a plan of care, with input from you and your family members.
Q. What is the average length of stay for short-term rehabilitation patients?
A. Every patient is unique and so is their length of stay. Your physician will give the best recommendations to get you or your loved one back on their feet as quickly as possible.
Q. Are utilities, phone, internet, and cable included?
A. Yes, most are included. Our Transitional Care guests receive all utilities, cable, internet, and phone as a part of their daily rate. For our Long Term Care residents, all utilities, cable, and internet are included; phone services are available for an optional fee.
Q. Is Medical Assistance available if there is not enough insurance coverage to pay for a stay?
A. If you are qualified through the State of Minnesota, yes. If you are unsure whether or not you qualify, contact your insurance carrier or ask us to help you determine your eligibility.
Q. Are community tours offered?
A. Yes! We feel that tours are the best way for you and your family to learn about who we are, what we do, and how we do it. Visiting with us will give you a great opportunity to see our community, meet our staff, and learn what life will be like if you choose us for your short-stay or long-term care needs.
Q. Can an existing family doctor be the attending physician during a stay?
A. Yes, but we would need verification that the physician agrees to follow you while you stay in our community.
