Admissions
We understand that transitioning to a care community can often be overwhelming. At Accura HealthCare, our Admissions team is here to guide you through the admissions process to help make the transition as easy as possible.
Our Admissions teams work with you, your family, and your referring provider to review your history and learn more about your specific needs. We also assist with the coordinating the transition between the hospital, other health care settings, or your home into our community. Assistance with understanding your Medicare, Medicaid, or other insurance benefits is provided and we are here to help complete any required paperwork. Our goal is to make the process easier for you while providing the right amount of support and guidance that each individual family needs.
Making a Referral
Referrals are accepted 24 hours a day, 7 days a week!
Referrals are reviewed for appropriate placement by our Admissions teams and can be submitted via electronic submission, email, phone, or fax!
We accept referrals for our skilled nursing communities (transitional care, long-term care, respite services), memory care living, assisted and independent living from an inpatient setting, an emergency department, a doctor’s office, or from home.
